Job summary
Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Ocean County, NJ. Flexible schedule and both 1099 and W-2 options are available, with opportunities for professional growth and a supportive work environment.
The ideal candidate will be licensed in New Jersey (or eligible), with the ability to travel to patients’ homes within the assigned geographic area.
Qualifications
- Licensed Physical Therapist in NJ or eligible.
- Home health experience preferred but not required.
- Ability to travel to patients’ homes within the assigned geographic territory.
- Physical Therapist Program Graduate.
Responsibilities
- Accept and treat patients within your designated geographic area.
- Provide 1:1 treatment.
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
- Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
- Educate family members, home health aides, and other caregivers on proper therapy techniques.
- Plan and carry out occupational therapy programs to maintain, improve or restore physical function.
- Perform and document an initial exam and determine a diagnosis prior to intervention.
- Provide timely and accurate documentation for each visit.
- Evaluate effects of treatment and adjust accordingly.
- Willingness to treat geriatric patients.
