Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Per Diem Physical Therapist Assistant to provide in-home care under the supervision of a licensed Physical Therapist. Location includes in and around Valley Stream with a flexible schedule. Responsibilities include treating patients at home, following the PT's plan of care, and documenting progress

Qualifications

• Graduate of the PTA program and eligible for NY licensure.
• First Aid, AED, CPR certifications required.
• Home health experience preferred; mentorship provided.
• Ability to travel to patients' homes within assigned geographic area.

Responsibilities

• Treat patients in their homes under the supervising PT's plan of care.
• Monitor and report changes in patient status promptly.
• Communicate with patients, caregivers and team to ensure coordinated care.
• Instruct patients and caregivers in exercises and safety.
• Document treatments accurately and timely per standards.
• Assist with updating care plans based on progress.
