Job summary

Metro Physical & Aquatic Therapy seeks a motivated Physical Therapist Assistant to provide in-home care under the supervision of a licensed PT. This per diem role offers flexibility and the opportunity to work with a diverse patient base across Bergen County, NJ.

Qualified candidates are PTA graduates, licensed in NY or eligible, with First Aid/AED/CPR certifications. Travel to patients' homes is required, with mentor support and training provided as needed.

Responsibilities

• Accept and treat patients within geographic area
• Provide physical therapy treatments in patients' homes as per licensed PT plan of care
• Monitor and report changes in patient status to supervising PT
• Communicate with patients, caregivers and interdisciplinary team for coordinated care
• Instruct patients and caregivers in exercises, safety, and use of assistive devices
• Assist PT in updating care plans based on progress and response
• Document treatments accurately per agency standards
• Participate in care planning and discharge processes with PT
• Adhere to applicable regulations and agency policies

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care
• Monitor and report changes in patient status to the supervising PT in a timely manner
• Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care
• Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT
• Assist the PT in updating care plans based on patient progress and response to treatment
• Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards
• Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT
• Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice

• Physical Therapist Assistant Program Graduate
• Licensed Physical Therapist Assistant in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
