Assured Home Health, part of LHC Group and within the Optum family, is seeking a Per Diem Physical Therapist Assistant in California to perform therapy per the plan of care and educate patients and families. You will work with supervising PT and healthcare team to deliver quality home health care.

The role emphasizes adherence to state laws, patient safety, and adaptability to varied home settings. Weekend availability and CPR certification are preferred requirements.

Qualifications

• Current California license/registration to assist in physical therapy.
• Available to work weekend rotation.
• Ability to perform in home-health settings and follow plan of care.

Responsibilities

• Follow state laws and supervising PT directives in home care.
• Instruct patients in active and passive exercises and ADL tasks.
• Record patient responses and coordinate with care team.
• Educate patient and family on therapy activities and safety.
• Function effectively in diverse home environments.
