Job summary

Best Home Health Providers Inc. seeks a per diem Physical Therapist to support patient mobility and recovery in home health settings across California. You will assess functional status, implement plans of care, and educate patients and families on exercises and equipment.

The role requires an active California PT license, 2+ years of PT experience (home health preferred), CPR certification, and a reliable vehicle. Opportunities to convert to full-time based on performance are available.

Qualifications

• Degree in Physical Therapy and active California PT license.
• Minimum 2 years PT experience; home health background is a plus.
• CPR certification required; valid driver’s license with reliable vehicle.

Responsibilities

• Assess patients' function, mobility, and ROM to tailor plans.
• Deliver PT services per physician plan of care and CA Act.
• Educate patients and families on exercises, equipment, and recovery goals.
• Coordinate with physicians and care team; report changes and participate in conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is sole skilled service.
