Job summary

Best Home Health Providers is seeking a per diem Physical Therapist in California. You will assess patients, develop and implement treatment plans, and educate families to support mobility and independence in the home setting.

Responsibilities include collaboration with physicians, supervising support staff, and maintaining accurate documentation. A CA license and CPR certification are required, with strong communication and flexibility essential.

Qualifications

• Active California Physical Therapy license is required.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician’s plan of care and California Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is the sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
