Job summary

BAYADA Home Health Care is seeking an Occupational Therapist (OT) for a per diem opportunity to perform home health visits in Montgomery County, Maryland, serving adults and geriatric patients.

The OT will evaluate functional status, develop individualized plans, educate patients and families, and document care using an EMR system. MD OT license and at least one year of OT experience preferred. Excellent time management and independence are essential.

Qualifications

• Current Maryland OT license.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and evaluations in EMR.
• Educate patients and family members on rehabilitative care and activities.
