Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for home health visits in the Anne Arundel County area, including Glen Burnie, MD. This role supports adult and geriatric patients on a per-visit basis within Northern Calvert County communities.

The OT evaluates functional status, delivers individualized interventions, and coordinates with a multi-disciplinary team to enable patients to live safely at home.

Qualifications

• MD OT license required.
• Home health experience preferred.
• Minimum one year OT experience.
• Graduation from an AMA/AOTA-accredited OT program.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status and ADL/IADL skills.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing in the multidisciplinary care plan.
• Educate patients and families on rehabilitative care for safety and independence.
• Document observations and interventions in EMR.
