Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a Part-Time or Per Diem role in Northern Baltimore County, Maryland. The position provides home health rehabilitative care to seniors with limited commute, servicing 1–2 communities.

Responsibilities include conducting evaluations, developing individualized plans, assessing home safety, and educating clients and families. Maryland OT license is required; early mornings/evenings or weekend shifts may apply.

Qualifications

• Maryland OT license is required.
• Minimum 1 year OT experience.
• Home health experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and update the OT care plan in coordination with nursing.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions using a tablet-based system.
