Omega Home Health Care Services Inc. seeks a compassionate Occupational Therapist to join our home health team on a per diem basis. The role provides rehabilitative services in patients’ homes to help regain independence and improve quality of life.

Candidates must be licensed in California and have experience in home health or related settings. Per visit pay varies by visit type, experience, and location, with travel within a designated region required.

Formación

• Must hold current and valid California OT license.
• Graduate of an accredited Occupational Therapy program.
• Minimum of 1 year experience in home health or similar setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with electronic medical records (EMR) systems is a plus.

Responsabilidades

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients’ homes per physician orders and company protocols.
• Instruct patients and caregivers in the use of adaptive equipment and daily living techniques.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with nurses, physical therapists, and social workers.
• Maintain accurate documentation in compliance with California state and federal regulations.
• Ensure patient safety and infection control in home environments.
