Job summary

Apploi is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis in California. You will conduct patient evaluations, develop treatment plans, and deliver therapy in patients’ homes according to physician orders and company protocols.

The role requires a CA license and experience in home health. You will document progress, educate patients and caregivers, and collaborate with nurses, PTs and social workers while ensuring safety and regulatory compliance.

Qualifications

• Licensed Occupational Therapist in California is required.
• Graduate of an accredited OT program.
• Minimum 1 year of home health or similar setting experience preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with electronic medical records (EMR) systems is a plus.

Responsibilities

• Perform comprehensive patient evaluations and create individualized treatment plans.
• Deliver OT services in patients' homes per physician orders and company protocols.
• Instruct patients and caregivers in the use of assistive devices and daily living techniques.
• Monitor progress and adjust treatment plans as necessary.
• Collaborate with a multidisciplinary team including nurses, PTs and social workers.
• Maintain accurate documentation in compliance with regulations and patient safety protocols.
