Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis. The role involves providing rehabilitative services to patients in their homes to help them regain independence and improve quality of life.

California licensure is required and prior home health experience is preferred. The position involves travel within a designated geographic region, flexible per diem scheduling based on patient needs, and pay per visit.

Qualifications

• Current and valid California OT license.
• Graduate of an accredited OT program.
• Minimum of 1 year home health or similar setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with EMR systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver OT services in patients’ homes per physician orders and company protocols.
• Instruct patients and caregivers in adaptive equipment and daily living techniques.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with California state and federal regulations.
• Ensure patient safety and adherence to infection control procedures in home environments.
