Job summary

Omega Home Health Care Services, Inc. seeks a compassionate Occupational Therapist to join our home health team on a per diem basis.

You will provide rehabilitative services to patients in their homes to help regain independence and improve quality of life, working within physician orders and company protocols. The successful candidate is licensed in California, has experience in home health or related settings, and is able to travel within a designated region.

Qualifications

• Must hold current California OT license.
• Graduate of an accredited Occupational Therapy program.
• Minimum 1 year experience in a home health or similar setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with EMR systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver therapy services in patients’ homes per physician orders and protocols.
• Instruct patients and caregivers in adaptive equipment and daily living techniques.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nurses, physical therapists, social workers and other team members.
• Maintain accurate, timely documentation in compliance with regulations.
• Ensure patient safety and infection control in home environments.
