Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis. You will evaluate patients in their homes, develop personalized treatment plans, and deliver OT services in the home setting.

A California license is required, with preference for clinicians experienced in home health and travel capability within the region. The role offers per visit pay and flexible scheduling, with emphasis on improving daily living skills

Qualifications

• Licensed to practice Occupational Therapy in California.
• Graduate of an accredited Occupational Therapy program.
• Minimum 1 year of home health or similar setting experience preferred.
• Valid driver's license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with EMR systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients' homes per physician orders and company protocols.
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with regulations.
• Ensure patient safety and infection control in home environments.
