Job summary

One Medical is seeking a per diem Primary Care NP or PA for Fairfield County, CT. You will see a mix of acute, chronic, and well visits in person, with occasional telehealth, and collaborate with in-office and virtual teams to deliver patient-centered care.

The role requires at least 2 years of primary care experience, CT licensure (or eligibility), and board certification in Family or Internal Medicine or eligibility with plans to obtain certification within a year.

Qualifications

• At least 2 years of primary care experience.
• Connecticut licensure or ability to obtain licensure in CT.
• Board certified in Family Medicine or Internal Medicine, or Board Eligible with plans to obtain board certification within 1 year of start date.
• Completed an accredited NP or PA program with national certification.

Responsibilities

• See patients with acute, chronic, and well visits.
• Conduct in-office and telehealth visits.
• Collaborate with in-office teammates and virtual clinical teams.
