One Medical is seeking an Advanced Practitioner (NP/PA) for an hourly, per diem clinical role across multiple Fairfield County, CT offices. The position involves seeing patients with a range of acute, chronic, and preventive needs, both in-person and via telehealth, with daily collaboration with on-site and virtual teams.

• Completed an accredited NP or PA program with a national certification.
• In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in outpatient primary care, or 1 year with fellowship/urgent care experience.
• State license in CT obtained by start date.

Responsabilités

• Seeing patients with acute, chronic, and preventive care needs.
• Conducting in-office and occasional tele-health visits.
• Collaborating with in-office and virtual teams through daily huddles.
• Utilizing clinical training and performing in-office procedures when needed.
• Willing to obtain additional state licensure and credentialing for virtual primary care in additional states.
