Qualifications

Responsibilities

• Assist in developing, implementing, and documenting individual treatment support plans.
• Provide role model and support to clients to help integration into communities.
• Assess clients’ physical and emotional status and report changes to supervisor.
• Ensure program site safety and maintenance; oversee daily living activities with clients.
• Perform crisis intervention as needed.
• Complete paperwork and logs including communication logs, community membership logs, chore schedules, time sheets and application forms.
