Job summary

UCI Health is recruiting for a Case Manager to lead high-level assessments and care planning for patients, coordinating with interdisciplinary teams to deliver safe, cost-effective care and smooth transitions. The role emphasizes patient advocacy, cultural competence and accurate documentation within the medical record.

The candidate will bring at least 5 years of clinical experience with case management (or be an eligible internal applicant with BSN), and must have strong English communication

Qualifications

• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties.
• Must possess complete understanding of acuity levels for specific patient population.
• Must demonstrate customer service skills appropriate to the job.
• Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
• Familiarity of Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist professional that allows for independent assessments as permitted within the scope of practice of the discipline.
• Computer literacy and proficiency
• All external candidates must have a Bachelor of Science in Nursing
• Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
• Ability to facilitate and lead interdisciplinary rounds
• Ability to establish and maintain effective working relationships across the Health System

• Plan, organize, and implement goal-directed care for self-care outcomes.
• Collaborate with interdisciplinary teams to coordinate care delivery.
• Advocate for patient self-determination and choices.
• Maintain cultural competence in evaluations and planning.
• Intervene to improve quality outcomes and care transitions.
• Ensure medical records reflect illness severity and are accurate.
• Interface with physicians to improve documentation quality.
