Job summary

UCI Health is seeking an experienced Registered Nurse with Case Management expertise to lead patient care coordination within an interdisciplinary team. The role focuses on high-level assessments, planning and coordinating transitions of care, with emphasis on patient self-determination and safe, cost-effective care delivery.

The incumbent will oversee utilization review processes and ensure quality documentation, leveraging InterQual criteria and collaborating closely with physicians and care

Qualifications

• Minimum 5 years of clinical experience and 1 year of previous Case Management experience.
• Current RN License.
• All external candidates must have a Bachelor of Science in Nursing.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.

Responsibilities

• Conduct high-level assessment and reassessment to set priorities and plan patient care.
• Collaborate with interdisciplinary team to coordinate delivery of care and transitions of care.
• Advocate for patient self-determination and choice; ensure accurate clinical documentation.
• Coordinate utilization reviews and monitor resource utilization across the health system.
