Job summary

UCI Health is seeking an experienced RN for a Case Management role to assess and coordinate patient care across an interdisciplinary team. The incumbent will set priorities, plan care pathways, and ensure safe, cost-effective outcomes while advocating for patient self-determination.

The position requires a BSN, an active RN license, and at least 5 years of clinical experience with 1 year in case management. Familiarity with regulatory requirements is preferred.

Qualifications

• Excellent written and verbal communication skills in English.
• Minimum 5 years of clinical experience and 1 year of Case Management experience or a UCI employee with BSN.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Current RN License
• Ability to manage multiple concurrent projects and maintain a workload
• Ability to facilitate and lead interdisciplinary rounds

Responsibilities

• Perform high level assessment and reassessment to set patient/family care priorities.
• Plan, organize, and implement goal-directed care and transitions of care.
• Coordinate care with an interdisciplinary team to ensure high quality, safe and cost-effective outcomes.
• Advocate for patient self-determination and maintain clear clinical documentation.
