Job summary

UCI Health in Irvine, CA is seeking an experienced Registered Nurse for a pivotal Case Management role. You will perform high‑level assessment and reassessment to plan, organize, and implement goal‑directed care, coordinating interdisciplinary teams and transitions of care to optimize outcomes.

The successful candidate will have 5+ years of clinical experience, 1 year in case management or BSN, maintain current RN license, and demonstrate strong communication, documentation, and knowledge of

Qualifications

Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
Current RN License
All external candidates must have a Bachelor of Science in Nursing
Familiarity with Joint Commission, CMS, CDPH and CCS requirements
Excellent written and verbal communication skills in English

Responsibilities

Perform high-level assessment and reassessment to set priorities, plan, organize, and implement goal-directed care.
Collaborate with an interdisciplinary team to coordinate delivery of high-quality, safe patient care and transitions of care.
Advocate for patient self-determination and cultural competence in evaluations and planning.
Ensure clinical information in the medical record clearly reflects severity of illness through physician interfacing to improve documentation.
Utilize approved decision support criteria for utilization review and intervene to enhance patient outcomes.
