Job summary

Lexington Health is seeking a Physical Therapist for outpatient services at Midlands centers. The role focuses on restoring function after trauma or illness, with a full-time day shift schedule (M–Th 7a–6p, Fri 7a–1p) and a $10,000 sign-on bonus.

Minimum qualifications include a Bachelor of Science in Physical Therapy and SC licensure. You will evaluate patients, develop treatment plans, document progress, and educate families, working with a multidisciplinary team in a Magnet-status system.

Qualifications

• Bachelor degree in Physical Therapy; SC licensure or eligibility
• Licensed to practice physical therapy in South Carolina
• No minimum years of experience required

Responsibilities

• Provide direct outpatient PT services per physician orders
• Assess patients using standard tests and measurements
• Develop and implement PT treatment plans
• Document patient progress and discharge summaries
• Instruct patients and families in home exercise programs
• Supervise PTA and collaborate with interdisciplinary team
• Communicate with physicians and care teams about plans of care
• Schedule patients and participate in department meetings
