Job summary

Lexington Health in West Columbia, SC is seeking an experienced physical therapist to provide outpatient services focused on restoring function after injury or illness. You will perform assessments, design treatment plans, and guide patients through therapeutic exercises and modalities in a fast-paced setting.

Qualifications include a Bachelor of Science in Physical Therapy, SC licensure, and CPR certification.

Qualifications

• Licensed in the State of South Carolina by the SC State Board of Physical Therapy Examiners.
• CPR Certification.
• Graduate of a PT program approved by the American Physical Therapy Association or equivalent.

Responsibilities

• Provides direct outpatient physical therapy to patients referred by a physician or APP.
• Evaluates patients using standard PT tests and measures (ROM, strength, neuro tests).
• Develops and implements individualized PT treatment plans.
• Administers treatments to relieve pain, restore function and maximize performance.
• Documents patient progress notes, evaluations and discharge summaries.
• Instructs patients and families in home exercise programs.
