Qualifications

• Must be in recovery from a severe mental illness or substance use.
• Valid driver’s license and insurability.

Responsibilities

• Performs professional case management for consumers with serious mental illness or emotional disturbance in a community setting.
• Arranges/provides services to promote recovery, independence, and community living.
• Coordinates delivery of mental health services with other agencies; monitors progress against treatment goals.
• Provides transportation as needed to access services and attend appointments.
• Attends multidisciplinary team meetings and maintains clear, organized documentation.
