Job summary

Mohave Mental Health Clinics, Inc. is hiring a health coach/peer support specialist to work with individuals enrolled in the clinic system. The role focuses on coaching, role modeling, and mentoring to help clients achieve independence and utilize available supports.

As part of a clinical team, you will support care plans, teach recovery tools, and help clients build natural supports while maintaining accurate documentation and meeting billing expectations.

Qualifications

• High School Diploma or GED is required.
• 21 years of age or older.
• No convictions for felonies or misdemeanors involving substance abuse, sexual misbehavior or assault.
• Flexibility with hours and own transportation.
• Must obtain fingerprint clearance in Arizona.
• CPR and first aid certification.
• Crisis to Care Intervention training within 30 days of hire.
• Peer Support Training with Recovery Competency Test and documentation.

Responsibilities

• Assist enrolled members in developing plans of care and identifying needs.
• Model recovery principles and provide coaching to clients.
• Teach and train recovery tools (WRAP, Recovery Planner, etc.).
• Assist clients in developing natural supports and community living.
• Demonstrate professional conduct and effective communication.
• Coordinate with supervisor to schedule activities, supervision time and training.
• Communicate progress with supervisor and clinical team.
• Complete required clinical documentation per agency, RBHA/health plan standards.
• Support plan development when peers provide services as needed.
• Maintain composure under pressure and meet deadlines.
• Provide customer service to internal and external stakeholders.
• Perform other duties as assigned.
