Axiom Community of Recovery (ACR) in Phoenix seeks a dedicated Peer Support Specialist to assist clients in recovery with emotional, social, and practical support. The role emphasizes empowerment, advocacy, and peer-driven care delivered through 1:1 and group sessions.

You will help navigate community resources, accompany clients to appointments, and ensure HIPAA-compliant documentation. Prior peer support certification and recovery experience are preferred.

Responsabilidades

• Provide ongoing encouragement and support to clients toward recovery goals.
• Complete biopsychosocial assessment and relapse prevention plan at admission.
• Facilitate psychoeducational groups.
• Arrange transportation and discharge planning.
• Intervene in crisis with de-escalation and safety planning.
• Apply CBT, motivational interviewing and trauma-informed care within scope.
• Educate clients on harm reduction strategies.
• Collaborate with departments to assess progress and barriers.
• Maintain HIPAA and CFR compliance; complete required documentation.
• Attend clinical supervision and treatment team meetings.
