Axiom Community of Recovery (ACR) is a Phoenix-based nonprofit that provides peer-driven recovery services and workforce development. This position supports clients through psychoeducational groups, assessments, safety planning, relapse prevention, and discharge processes under supervision.

Qualifications

• Bachelor’s degree or equivalent work experience in relevant field.
• Two years of behavioral health or substance abuse experience preferred.
• Lived recovery experience and active Peer Support Certification (preferred).

Responsibilities

• Provide ongoing encouragement and support to help clients reach recovery goals.
• Perform biopsychosocial assessments and relapse prevention planning.
• Facilitate psychoeducational groups and discharge planning.
• Handle crisis intervention and safety planning within scope.
• Maintain HIPAA-compliant documentation and coordinate with teams.
• Collaborate with departments to assess progress and barriers.
• Attend clinical supervision and treatment team meetings as required.
• Promote CLAS and language-access initiatives for clients.
