Job summary

Centene Corp. in Texas seeks an experienced care manager to assess and coordinate care for complex/high-acuity populations with primary medical needs. You will develop personalized care/service plans, educate members and families, and coordinate with providers and resources.

The role is hybrid with in-home assessments, requiring TX licensure and 4–6 years of related experience. Bachelor’s in Nursing preferred; RN license in TX required.

Qualifications

• Requires Graduate from an Accredited School of Nursing or a Bachelor's degree and 4-6 years of related experience.
• Bachelor's degree in Nursing preferred.
• License/Certification: State RN license in TX.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses ongoing long-term care plans and collaborates with care management team to identify providers and community resources.
• Coordinates between the member, family/caregivers and care provider team to ensure person-centered care.
• Monitors care plans and member status; revises plans to meet goals / needs.
• Reviews member data to identify trends and improve operating performance and quality care.
• Collaborates with healthcare providers to facilitate member services and revised care plans as needed.
• Provides and maintains all member information to ensure compliance with guidelines.
• Educates members and families on disease processes, resolving care gaps, and benefits.
