Job summary

Qualifications

• Must be licensed in TX as RN or NP.
• Bachelor's degree in Nursing; 4–6 years of related clinical experience.

Responsibilities

• Perform in-home care management assessments and coordinate services for high-acuity members.
• Develop and continuously assess long-term care plans/service plans with the care management team.
• Coordinate with providers, specialists, and community resources to address member needs.
• Educate members and families on disease processes, services, and benefits available.
• Monitor progress, revise care plans as necessary, and ensure compliance with regulations.
