Angels of Care Pediatric Home Health is seeking a Pediatric Transitional Nurse to provide 1:1 pediatric nursing in home settings. The role covers high-acuity patients, including trach and vent management, with emphasis on caregiver training and safe home transitions.

The HALOS Home Team offers flexible scheduling, on-call support, and a nurse-led culture. Requires active Georgia RN or LVN license and CPR certification. Local travel within 30–40 miles is expected.

Qualifikationen

• Active Georgia RN or LVN license.
• Current CPR certification.
• Experience with Trach care and Ventilator management.
• Ability to lift and assist pediatric patients.

Aufgaben

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergency situations and changes in patient condition.
