Job summary

UW Health’s American Family Children’s Hospital in Madison, WI is seeking a RN Navigator for Pediatric Complex Care. This 100% FTE role runs Mon-Fri, 8:00am-5:00pm, with no weekends or holidays. Hours may vary by department needs.

The RN will provide pediatric care coordination, education, and multidisciplinary team collaboration. The position requires a BSN with Wisconsin RN licensure, 3 years of clinical nursing experience, and preference for an advanced degree.

Qualifications

• BSN is required and Wisconsin RN license must be active.
• 3 years of relevant clinical nursing experience required.
• Ability to work in multidisciplinary teams and coordinate care.

Responsibilities

• Care for pediatric patients with chronic conditions, acute episodes, and behavioral health challenges while promoting prevention and wellness.
• Provide evidence-based nursing care for patients via telephone and MyChart.
• Assist with care coordination and clinic procedures.
