Job summary

Qualifications

• Current Massachusetts RN licensure.

Responsibilities

• Assist and/or complete initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
• Participate in multidisciplinary team assessments to determine the developmental needs of the child and the concerns and priorities of the family.
• Participate in the development of IFSPs with families and collaborate to identify strengths and resources.
• Provide services using a coaching model with children and families according to their IFSP.
• Monitor developmental milestones and progress with parents/caregivers to inform ongoing need for direct service, consultation and/or assessment.
• Provide parent/caregiver education on health and safety issues during home visits or parent education groups.
• Complete progress notes, reports, assessments, and correspondence as appropriate.
• Document all information necessary for client files upkeep.
• Participate in developmental screenings in the community.
• Attend program functions for clients and the public.
