Thom Child & Family Services in Norwood, MA is seeking a Registered Nurse to provide home- and community-based early intervention for infants and toddlers and their families. You will work in a coaching model to tailor services to each family’s priorities and cultural needs.

The nurse will support health, nutrition, safety, and developmental goals, collaborating with parents/caregivers to ensure meaningful, individualized care with strong teamwork and scheduling flexibility.

Formación

• Current Registered Nurse license in Massachusetts.
• Ability to work independently and as part of a team.
• Experience with pediatric home-based services preferred.

Responsabilidades

• Assist and/or complete initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
• Participate in multidisciplinary team assessments to determine the developmental needs of the child and family priorities.
• Develop IFSP with families and collaborate to identify strengths and resources.
• Provide services using a coaching model with home visits, child groups, and parent groups.
• Monitor developmental milestones and progress to inform service needs.
• Provide parent/caregiver education on health and safety during visits or groups.
• Complete progress notes, reports, and correspondence and document client files.
• Participate in developmental screenings in the community and attend program functions.
