Job summary

Chapters Health System is seeking a Pediatric RN Case Manager to assess and coordinate care for pediatric patients in home settings, ensuring plans reflect evolving needs and are implemented through the Interdisciplinary Team (IDT).

The role requires pediatric nursing experience, an active RN license, and EMR/EHR proficiency, with travel to patient homes and adherence to quality and safety standards. A valid driver’s license and BLS are required.

Qualifications

Current RN license in the state of employment.
Minimum of two (2) year pediatric nursing experience; hospice or hospital experience preferred.
Experience with EMR/EHR systems.
Valid driver’s license and automobile insurance per policy; reliable transportation for visit schedule.
Ability to visit participants in their homes to perform assessments.
Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

Provide direct care to patients and families as part of the Interdisciplinary Team (IDT).
Coordinate the Plan of Care with an individualized approach reflecting evolving patient needs.
Educate patients and families about disease, prevention, palliative care, and safety.
Conduct home visits to assess safety, medication and nutritional compliance, and DME usage.
Communicate changes in patient condition to IDT and other health professionals.
