Job summary

The role requires an active RN license, English communication skills, and experience with EMR/EHR systems. The position may involve home visits and travel as needed by patient care plans.

Qualifications

• RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Active BLS for healthcare professionals.

Responsibilities

• Provide direct care as part of the Interdisciplinary Team (IDT).
• Coordinate Plan of Care reflecting patient needs and IDT input.
• Educate patients and families about disease processes and palliative care.
• Visit patients at home to assess safety, medications, and needs.
