Job summary

Chapters Health System is seeking a Pediatric RN Case Manager responsible for comprehensive home-based assessment, care planning, and coordination with the Interdisciplinary Team to maintain patients in their home environments.

The role requires pediatric nursing experience, an active RN license, and EMR/EHR proficiency, with the ability to travel to patient homes and provide compassionate, expert care.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of two (2) year pediatric nursing experience; hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to visit Participant in their homes to assessments.
• Ability to perform the essential functions and physical requirements of the job, with or without reasonable accommodation.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, prevention, palliative care, and safety.
• Performs home visits to assess home safety, medication compliance, and DME needs.
• Communicates with physicians and IDT, SBAR as needed.
• Presents reports to IDT and collaborates with other disciplines.
• Supervises home health aides and CHWs as requested.
• May be required to report to work during emergencies to ensure continuity of services.
