Job summary

Chapters Health in Florida seeks an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to keep the participant in their home environment as long as possible.

The role requires an active Florida RN license, 1 year nursing experience (hospice or hospital preferred), EMR/EHR proficiency, a valid driver's license, and ability to visit homes for assessments.

Qualifications

• Current RN license in Florida.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• PACE participants require Alzheimer’s Disease and Related Dementias training per Florida Dept. of Elder Affairs.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance as per policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.
• Ability to perform physical requirements (lifting, bending, pushing/pulling).
• Active BLS for healthcare professionals (AHA or Red Cross).

Responsibilities

• Provides reassurance on the phone to patients and families and coordinates in-person visits when needed.
• Coordinates with support resources or personnel to resolve complex situations.
• Documents patient/family contact information in the EMR and communicates with the IDT.
• Completes initial and semi-annual assessments and explains services to patients/families.
• Initiates Plan of Care and coordinates nursing interventions to maximize comfort.
• Obtains physician orders and documents per policy.
• Represents the company at assigned facilities and facilitates referrals.
• Communicates with IDT members and uses SBAR for clinical updates.
• Supervises home health aides as requested.
• During emergencies may be required to report to a designated location to ensure continuity of services.
