Job summary

Chapters Health System is seeking an RN Case Manager to provide clinical, palliative, and supportive care to patients at home, coordinating the Plan of Care with the Interdisciplinary Team (IDT). This role emphasizes compassionate patient support, strong communication, and adherence to care plans to help patients stay in their homes longer.

Ideal candidates have an RN license, at least 1 year of nursing experience (hospice or hospital preferred), and BLS certification.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates the Plan of Care to reflect patient’s evolving needs.
• Educates patient, family and caregivers about disease, prevention, palliative interventions, and safety practices.
• Home visits to assess safety, medication compliance and living conditions.
• Documents patient changes and communicates with IDT; participates in IDT meetings.
• Supports referrals and collaborates with facilities; uses SBAR for communication.
