Job summary

Chapters Health System is seeking an RN Case Manager to assess and coordinate care for patients in their homes, collaborating with the IDT to keep patients safely in their home environment as long as possible.

The role requires an active RN license, at least 1 year nursing experience (hospice/hospital preferred), and proficiency with EMR systems; driving to patient visits is required.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Valid driver’s license and automobile insurance.
• Experience with EMR/EHR systems.
• Ability to visit participants in their homes and perform assessments.
• Excellent English communication (oral and written).

Responsibilities

• Provide direct patient and family care as part of the Interdisciplinary Team (IDT).
• Coordinate the Plan of Care with the IDT and update as needs evolve.
• Educate patients and families about disease processes, palliative care, and safety.
• Conduct home visits to assess safety, medication compliance, and equipment needs.
• Document care activities in the EMR and communicate with the IDT.
• Collaborate with facilities, physicians, and referrals to ensure seamless care.
• Respond to emergencies and adjust schedules as needed.
• Maintain professional conduct and comply with regulations.
