Chapters Health System is seeking a dedicated RN Case Manager - Pediatrics to assess patient and family needs, coordinate the Plan of Care with the IDT, and deliver clinical and palliative care in the home setting to maintain patient comfort. The role emphasizes teamwork with physicians, social workers, and other health professionals to ensure seamless, person-centered care.

The ideal candidate has pediatric nursing experience, an active RN license, and strong communication skills in English.

Qualifications

• Current RN license in the state of employment.
• Minimum 2 years pediatric nursing experience; hospice or hospital experience preferred.
• Experience using an EMR/EHR system.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English, both verbally and in writing.
• Ability to visit participants in their homes for assessments.
• Ability to perform the essential functions and physical requirements of the job.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patients and families about disease, palliative interventions, and care planning.
• Conducts home visits to assess safety, medication and equipment needs.
• Reports changes to IDT and participates in hospice referrals/admissions discussions.
• Presents concise reports to the IDT and maintains clear communications with physicians and staff.
• Supervises Home Health Aides as requested.
• During emergencies, may be required to report to work to ensure continuity of services.
