Job summary

Chapters Health System in Florida is seeking an experienced RN Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to enable home-based living.

You will conduct home visits, educate families, document in EMR, and collaborate with physicians to optimize comfort and quality of life, with occasional travel required.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• PACE location: Alzheimer’s Disease and Related Dementias Training.
• Experience with EMR/EHR systems.
• Valid driver’s license and auto insurance per policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Ability to visit participant in their homes for assessments.
• Ability to perform essential functions including lifting.
• Active BLS for healthcare professionals (AHA/Red Cross).

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patient, family and caregivers on disease, care and safety.
• Home visits to assess home safety and medication compliance.
• Reports changes in patient condition to IDT; initiates interventions.
• Participates with IDT to evaluate hospice referrals/admissions.
• Attends daily IDT meetings and communicates with team members.
