Job summary

H2 Homecare, an Atlanta-based pediatric homecare provider, seeks a dedicated RN Case Manager to coordinate patient care and liaise with families, physicians, and caregivers. The role emphasizes compassionate, compliant, high-quality pediatric home health services.

The ideal candidate is an organized RN with at least 2 years in home care, excellent communication, and a patient-focused approach to serve medically complex pediatric patients and support our clinical team.

Qualifications

• Active RN license in good standing.
• Minimum 2 years in home care.
• Pediatric home care experience preferred.
• GAPP experience preferred.
• Excellent communication and organizational skills.
• Ability to manage multiple patients independently.
• Compassionate, patient-focused care.

Responsibilities

• Conduct monthly in-home patient visits to assess needs and ensure quality of care.
• Complete patient admissions, recertifications, care updates, and required clinical documentation.
• Ensure compliance with company policies, state regulations, and clinical standards.
• Serve as the primary clinical contact for assigned patients and families.
• Collaborate with office staff to coordinate staffing and continuity of patient care.
• Provide clinical guidance and support to field staff as needed.
• Participate in care conferences and communicate updates with the interdisciplinary team.
