Job summary

Children's Health of Carolina, PA is seeking a Case Manager (RN) to coordinate patient care, advocate for families, and build strong provider relationships. You will develop care plans, assess needs, and connect patients with community resources to improve outcomes.

The role emphasizes collaboration across care teams, ongoing discharge planning, and timely documentation to support quality care and patient advocacy.

Qualifications

• Bachelor’s Degree in Nursing and an active RN license is required.
• Case management experience is required.
• Ability to work independently and collaboratively.
• Strong problem-solving, communication, and interviewing skills are essential.
• Knowledge of community agencies and resources is important.
• Proficiency with personal computers, email, and related software is expected.

Responsibilities

• Coordinate and facilitate patient care through assessment, planning, and implementation.
• Create care plans and assessments for patients, including those with Social Determinants of Health (SDOH).
• Communicate patient needs to families, providers, and the care team.
• Work with healthcare teams to develop and follow treatment plans.
• Build relationships with community resource leaders and represent the agency.
• Provide education on social and emotional factors related to care.
