Job summary

Help At Home, LLC is seeking an RN Case Manager in Georgia to oversee patient care plans, admissions, discharges, and scheduling across the home health team. You will educate patients and families, ensure regulatory compliance, and collaborate with billing to resolve coding and insurance issues.

The role requires an active Georgia RN license, CPR/First Aid, and at least 1 year of nursing experience. A supportive team and meaningful community impact await.

Qualifications

• Current Georgia RN license required.
• CPR/First Aid certification required.
• Minimum 1 year of clinical nursing experience.
• Ability to work professionally, ethically, and respectfully.

Responsibilities

• Monitor patient care plans, ensure compliance with medical regs and home health standards.
• Coordinate admissions, discharges and transfers with complete documentation.
• Work with field staff to assign cases, manage schedules and visits.
• Educate patients and families on services, plans, and meds.
• Contribute to quality control procedures and regulatory compliance.
• Collaborate with billing to review records, coding, and billing issues.
• Maintain accurate patient files and regulatory reports.
• Assist in policy updates reflecting best home health practices.
• Serve as primary contact for patient/family concerns and inquiries.
• Assist in training new clinical staff on documentation and protocols.
