Responsibilities

• Provides patient care.
• Evaluates patient condition.
• Establishes treatment goals in collaboration with patient and family and the treatment team.
• Develops written treatment plans and administers treatment.
• Delegates responsibilities to PT assistants and rehab technicians as appropriate.
• Re-evaluates patient status and revises treatment plan as necessary.
• Evaluates need for medical equipment and instructs on its use.
• Assists with discharge planning involving patient, family and community resources.
• Documents patient status and progress per established standards and reimbursement guidelines.
• Communicates with the interdisciplinary team to integrate physical therapy into overall patient care.
