Qualifications

• Graduate of CAPTE-Accredited Physical Therapy Program
• Licensed in Michigan
• CPR certified
• Member of the American Physical Therapy Association (APTA)

Responsibilities

• Provide patient care within licensed scope and state regulations
• Evaluate patient condition and establish interdisciplinary goals
• Develop and implement written treatment plans and progress notes
• Re-evaluate patient status and adjust plans as needed
• Coordinate discharge planning with team and family
• Instruct patients and families on home programs and equipment use
• Collaborate with inter-disciplinary team to integrate therapy
• Document progress and communicate with staff and stakeholders
