• Evaluate patient conditions and set interdisciplinary treatment goals.
• Establish and administer treatment plans; document progress.
• Assist with discharge planning and home program guidance.
• Educate patients, families, and students; participate in team meetings.
• Attend meetings and in-services; maintain patient confidentiality.
