Job summary

KidsCare Home Health is seeking a dedicated Physical Therapist to provide in-home pediatric physical therapy services. The therapist will assess, develop and implement treatment plans, educate families, and coordinate with care teams to optimize outcomes in a home setting.

Applicants should hold a Master’s in Physical Therapy, possess a state license, CPR certification, and have pediatric/home health experience preferred. A competitive benefits package accompanies this role.

Qualifications

• Master’s Degree in Physical Therapy required.
• Current state-issued professional license.
• Valid state driver license.
• CPR certified.
• Pediatric clinical experience preferred.
• Home health experience preferred.

Responsibilities

• Evaluate and conduct pediatric physical therapy in the home health setting.
• Direct patients and caregivers in home exercise programs and discharge planning.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient/family regarding progress and goals.
• Refer patients to appropriate services and resources as needed.
• Collaborate with physicians on patient progress and maintain records.
• Participate in quality initiatives and continuing education.
