Job summary

Qualifications

• Home health experience preferred.

Responsibilities

• Attend in-home pediatric physical therapy sessions and develop treatment plans.
• Coordinate care with families, physicians, and caregivers.
• Maintain documentation of patient progress and outcomes.
• Provide family education and home exercise programs.
