KidsCare Home Health is a pediatric home health provider seeking a Physical Therapist to evaluate and treat children in their homes. The role focuses on guiding patients and caregivers to improve function and independence, coordinating with the care team.

The ideal candidate has a Master’s in Physical Therapy, an active state license, a valid driver’s license, and CPR certification. Pediatric and home health experience are preferred, with opportunities for mentorship and professional growth.

Qualifications

• Master’s Degree in Physical Therapy is required.
• Current state license to practice physical therapy.
• Valid state driver’s license.
• CPR certification required.
• Pediatric clinical experience is preferred.
• Home health experience is preferred.

Responsibilities

• Evaluate, organize, and conduct pediatric physical therapy in home health settings.
• Direct patient and caregiver in tasks to restore, reinforce and enhance performance.
• Coordinate care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and home programs.
• Refer patients and families to appropriate services as needed.
• Document patient progress and maintain records.
